Impossible to miss. These are located on the first level.
S’pht Talk
Read the F’tha terminal
The terminal is located on Level 5 “Come Get Medicine” in the northern area.
Vid buoy
Missing underwater fusion gun on We’re Everywhere
This is in the 6th level. Skip the weapon and continue with the level until you get the achievement.
Shell Game
Take the elevator to the shotgun ammo in the southeast corner of We’re Everywhere
This is in the 6th level. Find a room that is near the 2x health charger.
Shoot the switch that is submerged in the water.
Be careful not to hit the sides of the switch or the room will be inaccessible.
Head to the northwest side of the level where the fusion gun was located.
Walk down the long hall until you end up in a room with switches.
Run over to the other platform and turn on either switch to raise it to the shotgun ammo room.
Citadel
Access the primary tower in the Citadel of Antiquity
Impossible to miss. Complete level 11. (I think level 11! maybe level 12)
Surf’s up
Achieve SPNKR on If I Had a Rocket Launcher… without firing any weapons or opening airlocks
This is at level 14 and is a little trickier.
When you enter the area with the first airlock, go back with the enemies.
The easiest way to get to the higher platforms is to make the bomb explode on you.
Once you get there, there will be an area with stairs leading to the SPNKR room.

Vale
Destroy the ultimate core logic center
This is in level 17 “Begging for mercy makes me angry!”
Impossible to miss. Beat the level.
Sleeping Beauty
Activate S’pht AI
Impossible to miss. Enter the last chapter.
Keep the oath
Turn off the pattern cache and charger in All Roads Lead to Sol
This is level 28, the last level.

Ensuring continuous mutual destruction
Kill the mother of all hunters by overloading the fusion gun
The “mother of all hunters” is a larger version of the normal hunter.
Hold the alt-fire button on the fusion gun until it explodes.
Try to be against the Mother of all Hunters during the last few beeps, she beeps 45 times.
